Inclusion of Spouse / Children / Parents as Dependents [Appendix 5]

Malaysia My Second Home (MM2H) participants who have not included their spouses or children under MM2H programme in the initial application may apply to do by submitting the following documents:

  • Letter of application by participant
  • IM 12 FORM (each dependant must fill in 2 copies)
  • Statutory declaration from the participant/sponsor
  • 2 passport size photographs of the dependant
  • A certified copy of Marriage certificate/birth certificate/legal documents (where is applicable)
  • A copy of the dependant’s passport (certification on the pages with the personal particulars)
  • A copy of the participant’s passport (certification on the pages with the personal particulars)
  • A copy of the MM2H Social Visit Pass endorsement of the participant
  • A stamped Personal Bond/Security Bond (where is applicable)
  • A copy of the sponsor’s identification card (if applicable). Approved applicant is notified 10 working days from the date of submission
